- Marriage or Divorce
- To change your name:
- Bring in your new Social Security card to Human Resources (HR)
- Fill out a Name Change Form
- To change your beneficiary designations:
- Life insurance and AD&D insurance: Fill out a new beneficiary designation form
- Retirement savings plan: Contact TIAA-CREF directly
- To change your Flexible Benefits Plan elections:
- Fill out new benefits forms within 30 days of your status change and turn them in to HR, CMB 1064
- Fill out an Affidavit of Marriage or Domestic Partnership form, if applicable, and turn it in to HR, CMB 1064
- To change your federal income tax withholding, fill out a new W-4 form and turn it in to Payroll, CMB 1008
- To change the financial institution to which your earnings are deposited, fill out a new Direct Deposit form and turn it in to Payroll, CMB 1064
- To change your emergency contact or partner information, home address, or telephone number, contact Human Resources at hr@pugetsound.edu or x3369

- Birth or Adoption of a Child
