If you are injured on the job, take the following steps:

  1. Notify your supervisor.
  2. Notify Human Resources.
  3. Fill out a SIF-2 Form and an Incident Report Form in the Human Resources office (Howarth 016) during business hours (M-F, 8 a.m.-12 p.m., 1 p.m.-4 p.m.).
  4. Submit all of your doctor's note(s) to Human Resources (Howarth 016, CMB 1064, benefits@pugetsound.edu) within 24 hours of your appointment(s).

Your workers' compensation claim will be managed by our third-party administrator, Eberle Vivian.

To Change Your Chosen Name and/or Pronouns:

To Change Your Legal Name:

  • Complete the Legal Name Change Form (Fillable PDF, Download to Edit).
  • Bring your completed form and your new social security card to Human Resources (Howarth 016) during business hours (M-F, 8 a.m.-12 p.m., 1 p.m.-4 p.m.).
  • Note: If you have a University of Puget Sound retirement savings plan account, you will need to notify TIAA of your name change.

From there, you have the ability to change the following information:

Benefits-eligible faculty and staff can enroll in health coverage online through Employee Navigator. Employee Navigator is where you can make and review your benefits elections or submit benefits changes due to a qualifying life event. The carrier for Puget Sound's retirement savings plan is TIAA. To make elective deferrals or to change to your contribution amount(s), visit tiaa.org/pugetsound to log in to your online account, or call TIAA at 1.800.842.2252.

For Your Life Insurance and AD&D Insurance:

You will need to update your beneficiary designations online through Employee Navigator.

For Your Retirement Savings Plan:

You will need to notify TIAA by updating your online TIAA account or by calling 1.800.842.2252.

To request a leave of absence, complete the appropriate leave request form and submit it to Human Resources (Howarth 016, CMB 1064, benefits@pugetsound.edu). A leave request form should be submitted prior to the leave's start date or as soon as possible.
