Steps to upload your Covid vaccination documents.
1 – Access the Communication Portal here.
2 – Select Medical Clearances from the left side of the screen.
3—While in Medical Clearances, select Immunization Record to upload the image of the vaccine card or other document.
Note: further down on the menu, there is an “Immunizations” section. You can view all your immunizations that have been previously entered and verified, but you cannot add any new ones. That has to be done following the steps above.

Please complete your health history and immunization form. A paper copy is found in the Forms section.
The electronic version is located on the CHWS Communication Portal. Here, you will log in and upload your immunization record, insurance card, and complete web forms.
Please note that we do not bill insurance companies for visits, but we use the information to help students with referrals for outside providers.
