To complete this application for the 2017-2018 academic year, you will need to answer multiple questions about your background, compose short essay responses, and submit the $60 nonrefundable application payment to the address at the bottom of this form. Read through the application and collect all your materials before you begin completing this form. You will not be able to save and return later.

All application materials must be submitted prior to March 1, 2017. Materials submitted after March 1 will be considered as space permits.

  • Test Scores

    Washington State requires candidates for teacher preparation programs to demonstrate proficiency in basic reading, writing, and mathematics skills using one or more approved standardized tests. Please indicate which of the following skills requirements you have met.

    NOTE: official test scores must be sent directly to the university, and should be received by June 15th, 2017. Candidates must have approval from the School of Education to test after this date. No candidate will be allowed to enroll in the MAT program without submitting passing scores for both basic skills and content area exams.

  • Disciplinary History

    Candidates who do not hold a valid Washington certificate will be required to complete the Character and Fitness Supplement as part of the application for Washington certification. This application includes Washington State Patrol and FBI fingerprint clearance.

    Candidates with any previous criminal convictions, serious behavior problems, or previous license revocation must be cleared by the Office of the Superintendent of Public Instruction prior to student teaching or internships.

    Upon admission to the program, you will be sent instructions for fingerprinting and completion of a Washington State background check.
