Recommendations and Evaluations

Required evaluations and recommendations can be submitted online through the respective graduate school applications. Offline submissions are accepted for some, but not all, programs. Please be sure to carefully review our requirements. Eligible materials can be sent offline via mail or fax to:

School of Education

Master of Arts in Teaching 

School of Education applicants must arrange to have two (2) Appraisal of Applicant for Degree Candidacy forms completed and submitted. Candidates should submit at least one academic reference if they have taken coursework at a college or university within the last three years. The PDF form can be completed and mailed or faxed, or candidates can invite evaluators to complete the form online through the School of Education application.

Master of Education in Counseling

School of Education applicants must arrange to have two (2) Appraisal of Applicant for Degree Candidacy forms completed and submitted. The PDF form can be completed and mailed or faxed, or candidates can invite evaluators to complete the form online through the School of Education application.

Occupational Therapy

Master of Science in Occupational Therapy

Letters of reference are requested and submitted through the Occupational Therapy Centralized Application Service (OTCAS). Recommendations cannot be submitted offline. 

Post-Professional Doctor of Occupational Therapy

Applicants for Post-Professional Doctor of Occupational Therapy Program are required to submit one letter of recommendation from an OT supervisor. The letter can be mailed or submitted electronically through the Post-Professional Doctor of Occupational Therapy application.

Physical Therapy

Doctor of Physical Therapy

Three letters of recommendation are required as part of the Doctor of Physical Therapy application. At least one letter must be from a physical therapist and one from a professor. Letters are requested and submitted through the Physical Therapy Centralized Application Service (PTCAS) and cannot submitted offline.
