    I acknowledge my active participation in the LOGGER2LOGGER mentorship program. The duration of the program is one semester with an opportunity to renew with mutual agreement. I understand that I represent myself and also the University of Puget Sound and strive to be a model representative of the university. I recognize that although the University of Puget Sound is facilitating this opportunity, I am responsible for any incidental expenses I may incur (travel expenses related to worksite visit, etc.). I acknowledge that the university shall not be liable for any direct or indirect, consequential or incidental harm or loss, including injury, property loss, accident, or delay. If, for any reason, and at any time, I am not able to fulfill an agreed upon mentor-mentee activity, I understand that it is my responsibility to contact my mentor and inform them that I will not be present on the agreed upon day and time. When requested, I will offer my candid feedback about benefits and limitations of the program.

    Managing Confidential Information:
    By signing this document and entering into this mentorship agreement, all parties agree to respect confidentiality and understand its boundaries and limitations. As part of the mentorship relationship, participants may have access to certain information not generally known to the public related to the university or mentor employing organization. We agree that the information is “confidential information” and is protected by law and agreed not to be disclosed.
