- Bring in your new Social Security card to Human Resources (HR)
- Fill out a Name Change Form
- To change your beneficiary designations:
- To change your Flexible Benefits Program elections:
- To change your federal income tax withholding, fill out a new W-4 form and turn it in to Payroll, CMB 1008
- To change the financial institution to which your earnings are deposited, fill out a new Direct Deposit form and turn it in to Payroll, CMB 1008

- Birth or Adoption of a Child
- To change your Flexible Benefits Program elections:
- Fill out new benefits forms within 60 days of a new birth and 30 days of a new adoption and turn them in to HR, CMB 1064
